You can apply for the registration of stay certificate if you are a UK national / the Withdrawal Agreement beneficiary and you do not have a registration certificate of an EU citizen issued in Poland by the end of 2020, but you resided here by the end of 2020 and continue your stay.

You can submit the application from 01/01/2021 to 31/12/2021.
In the case of a foreigner who is a minor, the application is submitted by parents or guardians appointed by the court or by one of the parents or one of the guardians appointed by the court.
Documents needed:
• application completed in accordance with the instructions in 1 copy).
• 4 photographs,
• a photocopy of a valid travel document (the original for inspection), in a justified case, when you do not have any valid travel document and you are not able to obtain one, you may present another document confirming your identity.
• a written declaration (signed) on the continuation of your stay in the territory of Poland after December 31, 2020 with an indication of absences exceeding 6 months in the 12 months preceding the submission of the application or other evidence confirming that you resided in Poland on December 31, 2020. or earlier (this could include an employment contract, a tenancy agreement or a utility bill in your name, dating from 2020 or other evidence of recent entry to Poland, such as tickets, etc.).
*Check which documents can confirm that you meet the conditions for stays exceeding 3 months. For stays up to 3 months before December 31, 2020 provide us with the date of last entry into Poland (also applies to persons previously staying in Poland), e.g. a signed declaration or travel ticket. There’s no fee to apply for the registration certificate.
If you are a worker or you have a job offer:
• employer letter confirming employment and evidence that the employer is genuine, a document confirming that you have been offered a position, issued not longer that a month prior the application date
If you are a self-employed person:
• written statement confirming registration with the National Court Register (KRS) oraz Central Register and Information on Economic Activity (CEIDG) or other evidence of your business activity
If you are a student or have a vocational training:
• letter or certificate from your school, college, university or other accredited educational or training organisation showing the dates you enrolled, attended and completed your course - issued not longer that a month prior the application date,
• document confirming comprehensive sickness insurance
• document confirming that you have enough money to live on such as a bank statement or account summary
If you are a spouse of a Polish citizen:
• marriage certificate, issued not longer that 3 months prior the application date,
• your spouse' identity document (a certified copy)
If you are a self-sufficient person:
• document confirming comprehensive sickness insurance
• document confirming that you have enough money to live on such as a bank statement or account summary
If you are a UK national and joining a UK national as a family member:
• a copy of registration certifcate of your UK family member you are joining
• evidence of your relationship with your family member
o spouse - marriage certificate, issued not longer that 3 months prior the application date
o child – birth certificate
o parent – birth certificate of the family member you are joining and a proof of self-sufficiency
